Duties The Patient Aligned Care Team (PACT) Registered Nurse (RN) is responsible for providing competent, evidence-based gender-specific primary care to patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the clinic setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
