Registered Nurse - Outpatient Mental Health South Jordan, Utah Summary The Outpatient Mental Health (OPMH) Registered Nurse (RN) executes position responsibilities to improve patient and organizational outcomes. The psychiatric Mental Health (MH) RN executes their position duties by providing patient centered and recovery oriented care and maintaining self awareness while caring for patients living with MH conditions. The OPMH RN facilitates care coordination, linking with the Inpatient MH Units to facilitate continuity of care. Duties A Career At VA Salt Lake City Health Care System Can Take You Places (youtube.com) Major Duties: Provides psychosocial support, assists with problem solving and development of coping skills, and supports the Veteran's personal goals for recovery. Obtains a health history of referred patients; data collection includes lifestyle patterns, pain issues, sleep/activity levels, support systems, cultural, social, and ethnic factors, patient treatment preferences and goals. Assesses patients and provides additional support between visits with licensed independent providers (LIPs). Refers to, consults with and coordinates care with other VA health care providers.
