Physician - Primary Care Medical Director Union County, South Dakota Summary This position is located in the VA Health Care System Community Based Outreach Clinic in Union County, South Dakota. Qualified applicants will be considered and referred every 2 weeks until the announcement closes on June 15, 2026. Duties Functions as the Medical Director and Physician for the Patient Aligned Care Team (PACT) within Ambulatory Care Services at the Sioux City Community Based Outpatient Clinic (CBOC) located in Dakota Dunes, SD and is part of the Sioux Falls VA Health Care System (SFVAHCS). The incumbent provides leadership, clinical oversight, and direct patient care in a rural outpatient setting, ensuring high quality, Veteran centered primary care aligned with VHA standards and organizational goals. MAJOR DUTIES & RESPONSIBILITIES The Medical Director serves as an integral member of the Primary Care team and performs the following duties: Clinical Care - Provides comprehensive primary care, including history and physical examinations, ordering and interpreting diagnostic studies, and developing treatment and follow-up plans. - Manages outpatient follow-up visits, post discharge care, and coordination with specialty and subspecialty services. - Delivers care through multiple modalities, including face-to-face visits, group clinics, telephone clinics, and secure messaging. - Ensures accurate and timely completion of patient encounters, documentation, and coding in accordance with VA guidelines. Care Coordination & Team Collaboration - Serves as the primary clinical point of contact for consultant services regarding patient care. - Provides consultative support to other Primary Care providers and services. - Assists with data
