Physician - Geriatrics Transitional Care Program Medical Director Richmond, Virginia Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Duties The Geriatrics Transitional Care Program Medical Director may practice in all settings, including inpatient, outpatient, and telehealth modalities. This physician may practice in all patient demographics, provide trainee education, participate in scholarly activity consistent with academic appointment, and implement several quality improvement projects. This position provides primary care to assigned panel of patients in the Patient Aligned Care Team (PACT)/nursing home. We are looking for a 1.0 FTEE Geriatric physician as medical director of the transitional care program. Join a Geriatrics & Transitional care program established and operational for ~ 20 years. One-site practice with geriatric transitional care, geriatrics clinics, PACT/nursing home, trainee education and quality improvement projects. Plan and implement next level expansion of Geriatrics Transitional Care program. Join a collegial group of physician and advance practice providers. Work with an engaged interdisciplinary team.
