Coordinate appointments, referrals, and telehealth services to ensure timely access to care. Maintain and update electronic health records, documenting patient interactions and care activities. Provide direct patient support including appointment reminders, education, and follow-up coordination. Support clinic operations through scheduling, reporting, and administrative functions.
