Registered Nurse - Women's Health Clinincal Navigator Biloxi, Mississippi Summary The Women's Health Clinical Navigator focuses on individual growth and development in practice with increased levels of self-direction. This nurse brings principles and theory with limited application of critical thinking skills. Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing. Duties The Women's Health (WH) Clinical Navigator will interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Anticipates and implements preventive patient care measures. Provides guidance and education to patients and caregivers and collaborates with other professionals at all levels. Supports the WH Program and evaluates care and outcomes to ensure timely and appropriate provision of gender specific services. Advocates for holistic care through patient/family education and provider interaction and implements an educational plan to meet the patient care needs. Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to: Coordinates care with multiple providers across all levels and sites of care. Addresses mammography, cervical cancer screening, bone density testing and treatment, maternity care coordination and reproductive health. Provides oversight of the patient's care through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled timely and completed. Monitors the provision of services and tracking of outcomes. Retrieves statistical data on care and utilization at the local VAMC using Veterans Health Administration (VHA) databases.
