About this role

Registered Nurse - Patient Aligned Care Team (PACT) - Care Manager San Diego, California Summary The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse Care Manager is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care. This nurse provides sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation. Duties The PACT RN Care Manager assumes the following major duties and responsibilities: 1. Provides direct patient care based on current standards of nursing care, PACT recommendations, patient care needs, and staff competencies. 2. Participates in PACT performance measure improvement activities such as, but not limited to, 2-day post-discharge contact, chronic disease management, preventative care, clinical reminder compliance, etc. 3. Participates in ongoing PACT process improvement workplace activities as appropriate to the individual's position, education, and practice environment. 4. Plans and documents patient care and nursing resource utilization, accounting of maximum staff productivity, staff/time efficiency, and positive patient outcomes. 5. Supports more than one PACT as an RN Care Manager when clinically indicated. 6. Participates in daily and weekly PACT huddles. 7. Performs unit audits as assigned. 8. Maintains current knowledge of and adheres to Primary Care standards, recommended practices and guidelines. 9. When available/offered, successfully completes Care Coordination and Transition Management (CCTM) and Telephone Triage training. 10. Completes TEACH and Motivational interviewing training within first year
