Registered Nurse - Community Care Women's Health Care Coordinator Johnson City, Tennessee Summary The Community Care Women's Health Care Coordinator (RN) is responsible and accountable for all elements of the nursing process when providing, coordinating, and/or supervising patient care and Women's Health community consults. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care and the referral process. Duties Functions or Scope of Assigned Duties: The Community Care Women's Health Care Coordinator (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and education for Veterans and their spouses, including but not limited to coordinating and managing community maternity care, gyn care, infertility treatment, in vitro fertilization, mammography, cervical cancer screening, and bone density testing. Adheres to VHA Directives governing treatment eligibility and community referral eligibility. The Community Care Women's Health Care Coordinator (RN) assists Veterans, spouses, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care including both VA and community providers. Coordination also includes assuring services interface within the VA and community programs to provide continuity of care to Veterans.
