Primary Care Provider (PACT Nurse Practitioner) Orem, Utah Summary The Primary Care Provider (Nurse Practitioner/NP) provides comprehensive clinical care, consultation, and patient management in support of the VA mission. They practice to the full extent of their training and certification, follow defined clinical privileges, and hold prescriptive authority for their assigned patient panel or program. Duties Start your career here. The Primary Care Provider (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Duties of the position include are but not limited to: Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement individualized plans to achieve optimal population health and disease management. Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members. Order and interpret labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiate referrals and consultations to specialty care services. Provide education to patients and their families. Assess and adjust the plan of care as needed to achieve optimal patient outcomes. Exemplify the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. Influence and model the delivery of evidence-based health care, demonstrate expertise in the ability to manage a patient panel, population, or program.
