PACT Licensed Vocational Nurse (Clearlake) Clearlake, California Summary The Patient Aligned Care Team (PACT) Licensed Vocational Nurse (LVN) at the Clearlake VA Outpatient Clinic provides direct patient care and clinical support to Veterans as part of the VA's team-based primary care model. The LVN delivers safe, compassionate, and patient-centered nursing care while supporting the coordination of services within the PACT model. The LVN plays an important role in promoting continuity of care, improving patient outcomes, and ensuring Veterans receive timely access. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.** This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
