Nurse (Patient Aligned Care Team, Outpatient) Salmon, Idaho Summary The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex primary care. This nurse provides sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Duties Salmon, ID is a scenic mountain-town located on the outskirts of Idaho's Frank Church River of No Return Wilderness, with rustic origins in mining, lumbering and family ranching. Over the years, it has evolved into a vibrant collection of art galleries, restaurants, and a community of adventurists that can only exist in a place that offers this much outdoor recreation. Functions or Scope of Assigned Duties include but are not limited to the following: Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
