Duties VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 This Licensed Practical Nurse (LPN) position is located within Primary Care at the New Albany VA Clinic, New Albany, IN, serving as a member of a VHA Patient Aligned Care Team (PACT), a patient-centered medical home model of care that is accessible, timely, coordinated, continuous, comprehensive, and compassionate. The PACT LPN performs non-standard and specialized Licensed Practical Nurse activities under the direction of a Registered Nurse, requiring a broad work experience and demonstrated skill sufficient to independently resolve a range of nursing care decision making with minimal periodic supervision by the Registered Nurse. Duties include but are not limited to: Provide full range of practical nursing care to patients with a variety of physical and/or behavioral problems, completing assigned duties which involve performing nonstandard and specialized practical nursing activities under the direction of the Registered Nurse. Complete patient focused assessments, recognizing risk factors and/or signs and symptoms of withdrawal. Complete clinical reminders and reports of data findings to clinician, provider, or RN. Administer medications and injections, observing and documenting patient response and reporting change in the patient's condition.
