Advanced Practice Registered Nurse - Home Based Primary Care Beckley, West Virginia Summary The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Home Based Primary Care (HBPC) within the Geriatric Extended Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides direct Veteran care and care coordination, as it pertains to Veterans and their care within the HBPC Program. Duties The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Core functions are outlined in the clinical privileges that correspond with this position. Major duties include, but are not limited to: Provide advanced clinical practice, consultation, and management. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Makes home visits to Veterans in the program. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies. Supports colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care.
