Nurse (Clinical Accountability & Metric - CAM) Chinle, Arizona Summary This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Chinle, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021). Duties Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Coordinates case management and care coordination services for high-risk and at-risk patients, including referrals, follow-up care, chronic disease tracking, and multidisciplinary treatment planning. Provides direct nursing care, patient assessments, triage, emergency interventions, and specialized clinical support for chronic, emergent, and complex patient conditions. Monitors patient progress through disease registries, laboratory and imaging follow-up, medication reconciliation, discharge planning, and continuity-of-care activities. Educates and counsels patients, families, and community members on disease prevention, treatment plans, self-care strategies, and available health resources using culturally appropriate approaches. Collaborates with multidisciplinary healthcare teams, Purchased/Referred Care, community agencies, and transportation services to improve patient outcomes and support quality improvement initiatives.
