Working closely with the population health teams, networked facility partners, primary care providers, and practice operations at Privia Health, the Nurse Care Manager will provide extensive care coordination for Privia's highest risk patients. Manage a diverse caseload of patients, ensuring timely and efficient follow-up on care plans, appointments, and treatment outcomes to optimize patient health and satisfaction. Build trust and rapport with a select group of Privia patients to reduce high-cost utilization and enhance clinical outcomes through telephonic and in-person engagement.
