Clinical Provider Engagement Partner - Michigan (RN or Case Manager) Grand Rapids, MI Porter is hiring a Clinical Provider Engagement Partner to join the Clinical Operations Team! Candidates must reside in Michigan. This is a hybrid position with travel required. (Field Based) Who We Are Porter exists because the most important moment in a patient's care often happens outside a clinic. Our Nurse Practitioners go into homes across Michigan and do what a fifteen-minute office visit never can, seeing the whole person, identifying what's been missed, and making sure that information gets to the providers who need it.
