About this role
Nurse (Case Management) Browning, Montana Summary A housing subsidy of up to $900 per pay period may be available. This position is located at the Blackfeet Service Unit, Division of Nursing Services, Emergency Room. The primary purpose of this position is to provide specialized case management, care coordination, and direct emergency nursing care for patients with complex health care needs served through the Emergency Department (ED). A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021). Duties Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) May cover Emergency Department nursing shifts during periods of staffing shortage or surge Perform ongoing assessment of patients requiring care coordination, including patients who do not keep appointments, leave against medical advice, or present frequently to the Emergency Department (ED). Collaborate with ED providers and the Resource Management Team (RMT) to determine medical necessity and level-of-care appropriateness using CMS-approved criteria for inpatient and outpatient referred care. Participate in case management conferences across disciplines, families, and outside agencies; serve as patient and family advocate throughout the continuum of care. Collects and maintains case management data, including service utilization, identified barriers, interventions, and patient outcomes. Use patient data to identify care gaps, improve coordination processes, and support quality improvement efforts.
