About this role
Social Worker (Mental Health Consultant) Spangdahlem, Germany Summary About the Position: Incumbent provides a team-based approach to healthcare to deliver outstanding, evidence-based biopsychosocial assessment, treatment, prevention and management of mental health problems and bio psychosocially influenced health conditions under the Primary Care Mental Health (PCMH) Program. This is a Direct Hire Solicitation Duties Screens, assesses patients, and performs PCMH model appropriate biopsychosocial evaluations. Conducts brief assessments and formulates evidence-based interventions appropriate to a primary care setting for a wide range of mental health conditions, chronic medical conditions, and health behaviors affecting medical conditions. Assists with implementation and monitoring of treatment plans with Primary Care Managers (PCMs) and other primary care team members. Provides ongoing consultation services for a subset of patients who would benefit from periodic, regular mental health monitoring and intervention for sustaining improvement and preventing relapse. Develops, teaches, and provides oversight for classes that promote education and skill-building to enhance psychological and physical health, and promote mental change related to improved health status.
