Physical Therapist UNAVAILABLE, UNAVAILABLE, UNAVAILABLE Where You'll Work St. Luke's Health - Patients Medical Center provides inpatient and outpatient medical and surgical services to residents of Pasadena, Deer Park, La Porte, Baytown, and Clear Lake. The facility includes 53 medical/surgical beds, eight ICU beds and three endoscopy rooms, and offers a range of primary and specialized services-wound care, general surgery, gastroenterology, occupational health, heart and vascular, women's services, diagnostic imaging, outpatient rehab services, and sleep disorders. Job Summary and Responsibilities Provide physical therapy services to patients/clients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other cause; Provides prevention and wellness services, including screenings and health promotion.. 1. Develop and update appropriate plan of care in collaboration with the patient/family and related to the persons age and lifestyle with measurable long and short term goals and a defined D/C plan Understand the role of physical therapy in the context of the patients age, total needs perspective and environment 2. Perform age appropriate competency skills in area of practice and with respect to individual patient problems. Follow intervention protocols and alter plan of care appropriately.
