Job Summary and Responsibilities CommonSpirit Health at Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time. As a Nurse Practitioner with CommonSpirit Health at Home, you'll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient's continuing eligibility for palliative care. Responsibilities: Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility. Establishes a plan of care for continued needs based on the client assessment. Reports changes in patient condition to appropriate personnel and the attending physician. Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.
