Navigator RN UNAVAILABLE, UNAVAILABLE, UNAVAILABLE Where You'll Work CHI St. Alexius Health Dickinson, part of CommonSpirit Heath, a national nonprofit health system based in Englewood, Colorado. Additional services offered within the system are: long-term care, assisted and residential living communities, community health services organizations, home health agencies, and numerous outpatient facilities. Job Summary and Responsibilities As our RN Clinic Navigator at CHI St. Alexius Health Dickinson, you will help patients and their families seamlessly navigate their healthcare journey so they can access comprehensive care and achieve optimal health outcomes. Every day you will coordinate care, educate patients, and connect them with vital resources and support. You will be expected to facilitate interactions between patients and their clinical care team, empower patients with information about their conditions and treatments, and collaborate with providers to implement care standards and secure appropriate services. To be successful in this role, you must possess strong collaborative and communication skills, a compassionate approach to patient care, and the ability to comprehensively assess patient needs and connect them with vital resources.
