As a Hospice Home Health Aide, you are more than a caregiver - you are a cornerstone of comfort, providing essential support that allows our patients to live their final chapters with dignity in their own homes. As a Hospice Home Health Aide, you will provide essential, hands-on care, ensuring dignity and well-being for our patients. Under the guidance of a Registered Nurse, your daily impact will include: Personal Care & Comfort: Assisting with personal hygiene, mobility (ambulation and transfers), and promoting overall patient well-being. Patient Monitoring: Diligently reporting any changes in condition to the nursing team. Supportive Environment: Performing light housekeeping, preparing meals, and ensuring a safe, healthful, and comforting home environment.
