We do this by working with patients to design a recovery program to meet their needs and ensure maximum benefits to their health. We are seeking a dependable, detail-oriented Patient Engagement Specialist to provide exceptional support to our patients and providers to ensure sessions are completed without any issue. Responsibilities - Schedule patient sessions and provide reminders about upcoming sessions - Complete task tickets as assigned to ensure patient schedules are accurate - Outreach patients at risk for churn in the program. - Help identify engagement strategies for patients throughout their program journey with Aviary Health - Communicate with providers and other care team members about patient scheduling delays or changes - Engage with patients using chat system, email, video conference, and phone - Educate patient about program offerings and use of the Aviary Health app - Troubleshoot any session technical issues that may arise, this includes supporting patients in joining virtual Zoom sessions from a Aviary Health-issued
