About this role
Care Navigator (On-Field) 1600 Corporate Center Dr., Monterey Park, CA 91754 We are seeking a highly engaged and patient-focused Care Navigator/Field Support professional to support the In-Patient Case Management team. This role will report directly to the Director and Senior Manager of In-Patient Case Management and will serve as a key liaison between patients, providers, and case managers to help ensure continuity of care, timely coordination of services, and positive patient outcomes. The ideal candidate is compassionate, highly organized, adaptable in fast-paced healthcare environments, and committed to delivering exceptional patient-centered support both telephonically and in the field. You are: - Compassionate with patients' various needs - Action-oriented with strong follow-through and problem-solving abilities - Collaborative and able to build strong relationships with patients, providers, hospitals, and internal teams - Resourceful and proactive in identifying barriers to care and escalating concerns appropriately - Mission-driven and passionate about improving patient outcomes and experiences - Organized and capable of managing multiple priorities in a fast-paced environment - A strong communicator with excellent customer service and interpersonal skills - Comfortable working independently in both office and field-based settings
