About the Role The Clinical Health Advocate (Licensed Practical Nurse) is an integral part of our care team, owning patient relationships and quarter backing a collaborative care plan that drives meaningful impact in our patients' lives. We're looking for a teammate who is an innate people person, loves helping others, and has a knack for tenacious problem solving. This is a part-time (up to 24 hours/week) hybrid role.
